I received a DSHS letter, requesting verification that my daughter is still attending school. Since she will be turning * yrs old on */*/*.
And that I must submit this information by */* or my benefits may stop.
I’m not able to get the requested info submitted by the deadline date. And I am requesting more time to get this info submitted.
The letter was printed on */*, the envelope postmarked on */*. And I didn’t receive it until Nov 4th. Even had I received it immediately after it was mailed, it would’ve only allowed me 10 days to do so.
I do have a Student Verification Form that is filled out and signed by my daughter’s teacher. It was required by Seattle housing authority.
It would be great if I could just submit this form, as it’s already filled out and signed. But I’m not sure if it meets all the requirements. I will attach the form, and if you would please let me know if it’s acceptable as other proof/ documentation.
If it’s acceptable, would you please send me the link/code or whatever so I can email the document to DSHS. As it’s the only way I’m able to submit that document at this time.
Also, we had pending legal eviction, writ of restitution. A signed, legal/ court ordered agreement to be moved out/ vacated from our current residence by */*/*. We were unable to find housing and don’t have family, relatives, friends or anyone else we can stay with. And we couldn’t afford to get our belongings moved out/ removed from the house either. So we are still staying at our current address. We are unsure if/ when we will be served the official notice to vacate, and/or be physically removed from the property by the sheriffs. I will notify you/ DSHS of any changes to our situation.

I have a Student Verification Form completed and signed by my daughter’s teacher, initially required by the Seattle Housing Authority. I am unsure if this form meets all the requirements. I will attach the form for your review. Please confirm if it is acceptable as proof/documentation.
If acceptable, please provide the link or code for submitting the document to DSHS, as email submission is currently my only option.
