I just got a letter with a date / time for a Fair Hearing that I did not request for.

Not in NJ but in NYC. The issue is that I've provided Home Care Services to my father in March which turned out that Agency was out of network, and Healthfirst sent a single case agreement form in which the Agency accepted. Healthfirst approved those services and then a few months later I started getting denial notices of those services I provided. My guess is either the Agency did not file on time or completely, but I am not sure.
